Transferring your medical records
If this is your first visit to HFA and you want the doctor to have your medical history, you need to have your medical records sent to us before your appointment.
To have your records sent, you must first sign a release form. Do this immediately after you make your appointment at HFA. You can do this in one of two ways, either:

Sign a release at your current primary care clinic and indicate that your
records be mailed to the address below, or

Download HFA’s Authorization to Release your medical records.
Print the form and complete all sections. Sign and date the form and mail it to your primary care provider’s release of information/medical records department. Indicate that your medical records should be mailed to:
