Insurance

HFA clinics accept a number of insurance plans. However, you must call your insurance company before your visit to HFA to make sure that the visit will be covered.

Always bring your insurance card with you, and also a check or cash. If your insurance company requires a co-payment, please make it during your visit.

If you do not have insurance, please call Patient Accounts at 612-347-5001 to set up a payment plan.

Referrals

Your insurance company may require you to designate a primary-care provider. This person may be a clinic physician, a nurse practitioner, or any other health care professional. He or she takes responsibility for your overall health and helps organize all aspects of your medical care, including referrals.

You may need a written referral from your primary care provider in order to see a specialist or have laboratory, x-ray, or other services. If this need arises, always be sure to speak with your insurance company and your primary care provider.

Referrals are usually written for a certain time span and/or a specific number of visits. It is important to keep track of this information to ensure insurance coverage.

Pre-authorization

Some procedures or medications may require pre-authorization from your insurance company. If necessary, your provider will obtain this. You can help us by supplying correct information to clinic staff.

If you are uncertain about specific contract requirements or restrictions, please let us know in advance.
