Patient Concerns

To those who entrust us with their care, we offer health, healing and comfort. Please tell us about your clinic experience and how we can serve you better.
Patients may obtain and fill out an HFA Comment Card from their clinic with a concern or compliment (or both), and either mail the card or deposit in a box which is provided by each clinic. These cards are then forwarded to Clinic Administration for review by the Director of Clinics. All concerns and compliments are entered into a database.
Patients may also call the Clinic Director. The phone number is listed on the Clinic Patient Complaint Form (Form HFA 457).
